ABSTRACT Background: Coronavirus (CoV) disease (COVID)-19 poses difficult situations in which the ethical course of action is not clear, or choices are made between equally unacceptable responses. Methods: A web search was performed using the terms “bioethics; COVID-19; ethics; severe acute respiratory syndrome CoV-2; emergent care; pandemic; and public health emergencies.” Results: Protection from COVID-19 has resulted in the cancellation of necessary medical interventions, lengthened suffering, and potential non-COVID-19 deaths. Prolonged lockdown reduced well-being, triggering or aggravating mental illnesses and violence, and escalated medical risks. Collateral damage includes restrictions on visitations to hospitals, alienation from the deceased relative, or lack of warm caring of patients. Finally, in a public health crisis, public health interest overrides individual rights if it results in severe harm to the community. Conclusion: Balancing ethical dilemmas are one more challenge in the COVID-19 pandemic. (REV INVEST CLIN. 2021;73(1):1-5)

ABSTRACT

Resumen: Los Comités Editoriales de revistas de corriente principal se ven enfrentados ocasionalmente a con ductas éticas inapropiadas en los manuscritos recibidos. El Comité de Ética en las publicaciones (COPE) ofrece recomendaciones para los editores respecto a cómo actuar frente a la sospecha de falta de ética en los manuscritos, ya sea recibidos o publicados. Cuando se pesquisa una mala práctica durante el proceso de revisión por pares, el manuscrito es rechazado, no obstante, si la conducta ina propiada es detectada después de la publicación de manuscrito, se procede a retractar la publicación. Revista Chilena de Pediatría no ha sido exenta a este tipo de conflictos. En este artículo analizamos los distintos aspectos relacionados con la falta de integridad de las publicaciones, como son las autorías, el plagio y el conflicto de intereses. Podemos concluir que las malas prácticas ocurren principalmente por desconocimiento de los autores, más que por intención de fraude. Se espera que el presente ma nuscrito logre instruir y sensibilizar a nuestros investigadores, respecto a las buenas prácticas en la investigación y publicación, y, contribuir, en lo posible, a prevenir que estas acciones ocurran en los manuscritos enviados a nuestra Revista.


Abstract: Editorial Boards of mainstream journals occasionally face ethical misconducts in received manus cripts. The Committee on Publication Ethics (COPE) provides recommendations for editors on how to deal with suspected ethical misconduct in either received or published manuscripts. The manus cript is rejected when malpractice is observed during the peer review process, however, if the mis conduct is detected after the publication, the publication will be retracted. The Revista Chilena de Pediatría (Chilean Journal of Pediatrics) has not been exempt from these type of conflicts. In this article, we analyze different aspects regarding the lack of integrity in publications, such as authorship, plagiarism, and conflict of interest. We can conclude that malpractices take place mainly due to the lack of knowledge of the authors rather than intent to defraud. It is expected that this article will suc ceed in instructing and sensitizing our researchers on good practices in research and publication, and contribute, as far as possible, to prevent this actions in the manuscripts sent to our Journal.

ABSTRACT

Ethical problems have always been present in scientific publications. Since the founding of medical journals, in the XIX Century, until today they are a source of concern because one main purpose of medical scientific publications is to add new, reliable information that could guide or modify medical decisions and public health policies. Since 1997, Revista Médica de Chile has published several articles clarifying this situation and emphasizing the need to avoid ethical misbehavior. The present review reminds that the main sources of information dealing with publication ethics appear in the web sites of ICMJE, COPE and WAME. Misconduct have been detected in Revista Médica de Chile in a few cases of redundant publications, plagiarism, lack of recognition of conflicts of interest mainly with pharmaceutical companies, and one attempt of forging the publication of an article that had been previously rejected. In handling situations identified as ethical misbehavior, the editors of this journal have successfully followed rules established by COPE. This article reviews and reinforces recommendations to avoid ethical misbehavior in biomedical research and in manuscripts submitted for publication.

ABSTRACT

Abstract: The case report is a special type of scientific publication that focuses on a single patient, raising problems of confidentiality, as the exposure of the intimacy may facilitate identification of the participants. The legitimacy for the public disclosure derives from the informed consent, ensuring the preservation of patients' self-determination. In this article, we discuss aspects of autonomy as basic ethical principle, framing under the Portuguese Law. In the case reports, as in any clinical investigation, the will of the patient should prevail over the interests of research and researchers, even when he is unable to consent, like the minors, the disabled or the deceased, enforcing the legal rules and addressing to the active involvement of the guardians and the relatives.


Resumen: El caso clínico es un tipo especial de la publicación científica que se centra en un solo individuo. Es ampliamente utilizado en la comunicación médica, tanto desde un punto de vista científico como pedagógico. Plantea problemas respecto a la confidencialidad en la medida en que la descripción de los detalles íntimos puede conducir a la identificación de los pacientes. La legitimidad de la presentación pública se deriva del consentimiento informado, que garantiza la preservación de la autodeterminación del paciente en cuestión. En este artículo, se discuten aspectos de la autonomía como principio ético básico a la luz de la legislación portuguesa. En los informes de casos, como en cualquier investigación, la voluntad del paciente debe prevalecer sobre los intereses de la investigación y de los investigadores, incluso si no puede consentir que en el caso de menores de edad, incapacitados o fallecidos, donde se asumen importantes cuestiones jurídicas y papel de los tutores y familia en este proceso.


Resumo: O relato de caso é um tipo especial de publicação científica que se centra num único indivíduo. É muito utilizado na comunicação médica tanto do ponto de vista científico como pedagógico. Levanta problemas quanto à confidencialidade na medida em que a descrição dos pormenores íntimos pode levar à identificação dos doentes. A legitimidade para a apresentação pública deriva do consentimento informado, que garante a preservação da autodeterminação do doente envolvido. Neste artigo, discutimos os aspetos da autonomia como princípio ético basilar, à luz da Lei Portuguesa. Nos relatos de casos, como em qualquer investigação, a vontade do doente deve prevalecer sobre os interesses da investigação e dos investigadores, mesmo se incapaz para consentir como no caso dos menores, os incapazes ou os falecidos, onde assumem importância as questões legais e o papel dos tutores e familiares neste processo.

ABSTRACT

El III Encuentro Nacional de Publicación Científica en Ciencias de la Salud se desarrolló del 29 al 31 de marzo del año en curso, auspiciado por el Centro Nacional de Información de Ciencias Médicas y la Universidad de Ciencias Médicas de Santiago de Cuba y tuvo como tema central la Política Editorial en revistas biomédicas. Con la participación de directores y editores de una amplia representación de las revistas biomédicas nacionales, y dos invitados internacionales cuyas conferencias resultaron enriquecedoras, el taller marcó las pautas del trabajo editorial en lo adelante, en cuanto a los objetivos comunes a alcanzar para garantizar la calidad y visibilidad de las mismas en nuevas bases de dato.


The 3rd National Meeting for Scientific Publication in the Health Sciences was held from 29th to 31st of March this year, sponsored by the National Information Center of Medical Sciences and the Medical University in Santiago de Cuba had as main topic the editing policy in biomedical journals. With the participation of directors and editors of a wide representation of the biomedical journals, and two international invited personalities, who presented enriching lectures, the workshop set the standards of the future editorial work, regarding the common objectives to be reached to guarantee their quality and visibility in new data base.

INTRODUCCIÓN: Los conflictos de intereses son condiciones en donde el interés primario es influenciado por uno secundario, comprometiendo la confianza en la investigación y difundiendo resultados sesgados. La declaración de conflictos de intereses (DCI) debe tener un fundamento claro, ser útil y parte de la estructura de toda investigación. OBJETIVO: Analizar la presencia de una DCI en las investigaciones publicados en la Revista Chilena de Ortopedia y Traumatología (RCHOT) y la Revista Chilena de Neurocirugía (RNC) en el periodo 2002-2015. MATERIAL Y MÉTODOS: Investigación descriptiva, retrospectiva y documental. Se llevó a cabo una revisión de trabajos publicados en la RCHOT y RNC, entre el 2002 y 2015. Los datos fueron analizados con el programa estadístico SPSS Statistics (versión 21). RESULTADOS: Se revisaron 596 trabajos, encontrándose DCI en 30,5%, siendo principalmente de financiamiento. La RCHOT presentó 70% de DCI, siendo en 2002 del 14% y en 2015 del 90%. La pertenencia del autor principal al comité editorial fue de 14%, y en un 73% de los trabajos, las instituciones de origen tenían representación en ese comité. Para la RNC se encontró un 2,3% de DCI. El 8,4% de los autores principales pertenecían al comité, y el 24% de las instituciones de origen de los trabajos tenían representatividad en el mismo. CONCLUSIONES: La DCI en la RCHOT fue irregular y centrada en el financiamiento. La DCI en la RNC fue casi nula. Es de esperar que esta investigación sea útil para los comités editoriales en el diseño de políticas y en desarrollar sensibilización en relación con una DCI.


INTRODUCTION: Conflicts of interests are situations in which a primary interest is influenced by a secondary interest. It could compromise the reliability of biomedical research and result in the dissemination of biased results. Declarations of conflicts of interests (DCI) must have a clear basis, be useful, and be part of the structure of all research. OBJECTIVE: To analyse the presence of the DCI in studies published in the Revista Chilena de Ortopedia y Traumatologia (RCHOT) and Revista Chilena de Neurocirugía (RNC), between 2002 and 2015. MATERIAL AND METHODS: A descriptive, retrospective and documental was performed by reviewing all papers published in the RCHOT and RNC between 2002 and 2015. All the information was analysed using the statistics program SPSS Statistics (version 21). RESULTS: After reviewing 596 papers, the DCI was found in 30.5% of them, mainly about funding. In RCHOT, 70% of the papers presented a DCI. In 2002 the DCI was 14%, and 90% in 2015. The principal author belonged to the editorial committee in 14%, and in the 73% of the papers, the institution of origin had representation on this editorial committee. In the case of RNC, 2.3% presented a DCI, 8.4% of the principal authors belonged to the editorial committee, and the 24% of the institutions of origin of the papers were represented on this committee. CONCLUSIONS: The presence of a DCI in RCHOT was irregular, and concentrated on funding aspects. The DCI in RNC was almost non-existent. Hopefully, this study will be useful for editorial committees in order to establish editorial policies and develop an awareness of the DCI.
